Joining the First Class Primary Care and Wellness Center direct primary care membership plan is an opportunity to save on your healthcare. Consider it buying in bulk – but for doctor visits. For an affordable monthly or annual fee, you get a certain number of office visits at any of our two north Florida locations, which saves you money over the per-visit cost of those visits.
What You Get
Our membership plan is available to veterans and nonveterans, whether you have insurance or are uninsured. It allows you to receive in-office primary care services at a reduced rate.
Membership includes:
- Annual wellness exam, including a physical examination, cancer screening, plus any applicable lab work and vaccinations
- Four additional office visits at any First Class Primary Care and Wellness Center location
What It Costs
There is a one-time registration fee of $50 to join the First Class Primary Care and Wellness Center direct primary care membership plan. Once you join, you can pay in one of the following ways:
- Monthly – $120/mo.
- Annually – $1,200/yr.
You will enjoy much bigger savings by paying the annual membership fee.
In addition, veterans receive a 20% off discount off the monthly or annual membership fee, making the cost either $108/month or $1,080/year.
